Perm - Physician - Geriatric Medicine Grand Rapids, MI

Trinity Health Medical Group (THMG) is seeking a full time (1.0 FTE) Geriatrics Physician to join our Geriatrics Team located in Grand Rapids, Michigan.

Job Description:

• Trinity Health Grand Rapids provides care for patients in eight local nursing facilities and supports the Trinity Health at Home Thrive Program, an Advanced Care Coordination Program.
• Opportunity to teach Residents, Medical Students, and Fellows

• Visit length adequate to meet complex patient population
• Call shared among Providers
• Open to new graduates who have completed a Fellowship in Geriatrics
• Open to experienced candidates in Internal Medicine or Family Medicine
• If candidate is TRULY Board Eligible, must become Board Certified within 2 years of becoming eligible
• Not accepting candidates who have lapsed Board Certification and need to obtain again
• Not open to commuters and/or those looking for secondary employment
