Perm - Nurse Practitioner - Palliative Care Findlay, OH

Job Description:

• 75% Inpatient Palliative Care; 25% Outpatient Palliative Care
• Delivering comprehensive Inpatient and Outpatient Palliative Care services at Lima Memorial Hospital (LMH)
• Conducting patient consultations for symptom management, chronic disease management, advanced care planning, and hospice transition discussions
• Leading delicate conversations with patients and families regarding terminal diagnoses and care goals
• Performing inpatient rounds for palliative care and hospice patients, including ICU rounds
• Managing outpatient palliative visits through Oncology department, infusion center, Comprehensive Health Clinic, and Telehealth
• Collaborating with hospital specialists (ICU, Oncology, Pulmonology, Cardiology) to align care plans
• Providing independent care in a fast-paced hospital environment with backup from Bridge team in Findlay as needed
• Maintaining documentation in Meditech (inpatient) and eClinicalWorks (outpatient)
• Serving as a clinical expert in palliative care, educating staff and advocating for patient-centered care
• Managing up to 6 patient encounters per day over a 5-day workweek
• Not accepting Locums candidates
  
• Not accepting new graduate candidates - experience required
