Perm - Nurse Practitioner - Hospice - Pallative Care Findlay, OH

Blanchard Valley Palliative Care is seeking a full-time Nurse Practitioner to join our compassionate and collaborative team providing outpatient hospice and palliative care services across Findlay, Ohio and surrounding counties.

Job Description:

• Provide outpatient palliative care services including home visits, extended care facility consultations, telehealth, and clinic-based care.
• Manage 6�8 patients per day based on location across a 9-county area in NW Ohio.
• Participate in delicate, goals-of-care discussions with patients and families facing serious or terminal illnesses.
• Assess, plan, implement, and evaluate individualized care needs in alignment with palliative care principles.
• Collaborate with a supportive interdisciplinary team including other NPs, physicians, and administrative staff.
• Participate in care transitions, including determining when patients would benefit from continued palliative care or hospice services.
• Work closely with a hospitalist team responsible for all patient admissions.
• Document patient encounters thoroughly and maintain excellent communication with patients, families, and care teams.
