Locum - Physician - Surgery - Hand Boise, ID

Saint Alphonsus Regional Medical Center is looking for a Hand Surgeon to fill their call coverage need!

Schedule:

• Call coverage: 1 or 2 days per month
• Start Date: August is not likely with credentialing, but please advise if your candidate is available for all open dates

Dates of coverage needed:

• July 30th
• August 6th, 7th, 20th, 28th
• September 2nd, 10th, 17th, 23rd
• October 7th, 14th, 30th
• November 5th, 6th, 17th, 26th
• December 4th, 10th, 15th, 17th

Please make sure you have the following?items readily available for your provider at the time of acceptance in order to expedite the credentialing process 

MD diploma 

Internship/residency/fellowship certs 

All medical licenses 

DEA 

Board Certification 

Life support cards- BLS 

Current and previous certificates of insurance 

Driver�s license 

Professional color photo/headshot 

CMEs for the past 2 years 

Immunization records including TB, Flu, HepB, Tdap, Varicella, and COVID 

References 

Case logs 

Travel Requirements:

• Mileage - IRS Standard Rate to be reimbursed by facility (only to be used with personal vehicle, not to be used with rental car).

Bill Rates Needed for Submission:

• Malpractice (must be included in the rates)
• Orientation
• 24 Hour On Call ( rate cap with 4 hours gratis)
• Holiday 24 Hour On Call 
• Call Back
• Conversion fee
