Locum - Physician - Radiology - Diagnostic Lakewood, CO

Summary:St. Anthony Hospital (SAH) in Lakewood, CO is looking for a Telehealth Diagnostic Radiologist to help with coverage for their Therapeutic Radiologist to cover GI Tumor Boards until their internal physician is able to start. Coverage needs are typically the 1st and 3rd Mondays of the month, but will be dependent on the Tumor Board meetings.

• Therapeutic Radiologist to cover GI Tumor Board
• Facility Location: 11750 W 2nd Pl, Lakewood, CO 80228
• Setting: Clinic
• Duties: Reviewing imaging and pathology to help stage disease and asses treatment options. Collaborating with other specialists (medical oncologists, surgeons, pathologists, etc) to create a cohesive, personalized treatment plan.

EMR Required Training:

• Module (learn lessons) � Must be completed prior to virtual EMR training to attend the training.
• EMR Virtual training � Only able to attend if Modules are completed before training. Virtual training must be completed at least one week prior to the providers first day. 
• All candidates must complete the following EMR questionnaire and detail it in the notes section of the candidates submission at time of presentation:
  • What are the # of years of epic experience they have?
  • When was the last time (MM/YYYY) that they used Epic?
  • What is the name of the last facility the provider used EPIC?
  • Can you provide me with a photo large enough to add to a badge and not become pixelated? - Please add photo into documents. 
  • Can you disclose the provider's time zone for scheduling the virtual epic training?
  • We need the provider's best phone # to use for call shifts while working. Can you disclose?
  • We need the provider's best email, to use for programs while working onsite and the preapplication. Can you please send us their personal email address?
